Why Claramedics Exists
Claramedics was created from a series of small moments that all pointed to the same problem.
Not a lack of care—
but a lack of clarity.
My mother-in-law was under the care of multiple providers—an oncologist, a primary care provider, and a neurologist. Each specialist was focused on their role, and over time, she was prescribed several medications, including pain medications from different providers.
What was missing was a clear, cohesive understanding of everything she was taking.
There was no single place where her medications were reviewed together. No list that she could show someone and say- "these are the medications I take." No simple explanation of what she should be taking, or how those medications overlapped.
She was under the impression that her various providers all had the exact same information. This is not always the case. Just because it is in the computers, does not mean all the systems talk to each other.
Like many older adults, she valued her independence and managed things on her own. But when her health began to decline, it became clear how little we truly understood about her medications—what they were for, how they worked, and what mattered most.
That experience was eye-opening.
And it's not uncommon.
The Reality of Care Transitions
Transitions of care often create the same confusion.
A patient may be discharged from the hospital with new medications, changes to existing ones, and instructions that feel overwhelming—especially for someone who lives alone. In an instant, a daughter or son may step into a new role as caregiver.
Now responsible for organizing medications, understanding what's changed, and making sure everything is taken correctly. But the information they receive is often rushed or difficult to interpret.
- What medication is new?
- What medication was stopped?
- What actually matters right now?
Without clear guidance, it's easy to feel unsure.

The Clinical Reality
Even from the clinical side, this gap is seen every day. When patients are admitted to the hospital, one of the first questions when the nurse admits them is "what medications are you taking?"
And many simply don't know: "I take a blue pill at night." "A small white one in the morning."
They're doing their best—but without a clear, accessible list, it becomes difficult to quickly and accurately identify medications. This leads to delays, frustration, and added stress—for both patients and care teams.

Providing Clarity and Confidence
Many older adults are organized, independent, and manage their medications well. But even then, having a clear, simplified explanation—and a clean, easy-to-read medication list—can make all the difference.
Something that can be kept in a purse or at home.
Something that can be handed to a nurse if needed.
Something that brings confidence, not uncertainty.
Claramedics was built to provide that. To take medication information and turn it into something clear, organized, and easy to read and understand. So patients feel informed. So families feel confident. And so no one is left trying to piece things together when it matters most.
